- Hospice Nurse Practitioner
Nurse Practitioner works with the multidisciplinary team to provide initial and ongoing clinical assessments of patients with terminal, advanced, and/or progressive health conditions. Job is a mix of both home based and facility based geriatric primary care, with emphasis on advance care planning, palliative management of symptoms, and aging in place. Job also includes integration with the hospice team to maintain continuity and performing visits for hospice patients to help meet face to face requirements, and symptom management visits when appropriate.
Qualifications:
- Must have a valid Driver’s License.
- ANCC or AANP board certified required in both Idaho and/or Oregon.
- If only licensed in one state, must be willing to obtain license for other state.
- Must have unencumbered valid license to practice.

- Hospice Social Worker - Fruitland
Hospice social workers provide care in coordination with the interdisciplinary team (IDT) to assess for initial support needs of hospice patients and families with adjustment to living with a terminal diagnosis, grief and bereavement support, psychoeducation related to end-of-life care, advanced care planning, and referrals to community services to support care and comfort of patients and families.

Work Location: Fruitland office serving patients in multiple counties
Job Duties:
· Complete bio-psycho-social assessment for new patients
· Develop care plan centered on patient and family needs identified in assessment
· Establish visit schedule as indicated by patients and needs
· Coordinate with interdisciplinary group to review care plans in accordance with CMS rules
· Conduct initial bereavement assessment to assess for identified bereaved
· Provide education on advance directives
· Assist in crisis management as needed with the IDG and patients
· Educate on housing or facility placement options and refer to community partners as needed
· Provide psychoeducation related to grief, loss, and end-of-life care
· Manage bereavement caseload
· Facilitate caregiver & grief support groups
· Provide supportive counseling related to grief and loss, psychoeducation for healthy coping, and bereavement
· Provide appropriate referrals to community resources
· Complete Military History Checklist to ensure veteran-centric care with assessment of needs
· Assist with other office or care-related tasks as assigned
· Complete documentation in accordance with agency protocol
· Adhere to state and local law, federal regulations, professional licensure regulations, and policy of Ancora Health Services
· Adhere to CDC guidelines and CMS regulations
Qualifications:
· Education: Masters Degree in Social Work preferred, or Bachelors Degree in Social Work from a CSWE-accredited school of social work, under appropriate supervision within the agency
· Hospice experience preferred
· License: Must maintain current licensure (LCSW, LMSW, LSW) based on state-specific rules
· Must maintain current driver’s license and auto insurance as specified by employer
· Willing to obtain Oregon license to assist in team coverage within 60 days of hire
