  • Hospice Nurse Practitioner

    Nurse Practitioner works with the multidisciplinary team to provide initial and ongoing clinical assessments of patients with terminal, advanced, and/or progressive health conditions. Job is a mix of both home based and facility based geriatric primary care, with emphasis on advance care planning, palliative management of symptoms, and aging in place. Job also includes integration with the hospice team to maintain continuity and performing visits for hospice patients to help meet face to face requirements, and symptom management visits when appropriate.

    Qualifications:

    • Must have a valid Driver’s License.
    • ANCC or AANP board certified required in both Idaho and/or Oregon.
    • If only licensed in one state, must be willing to obtain license for other state.
    • Must have unencumbered valid license to practice.

    Job Type: Full-time

    Pay: $100,000.00 – $150,000.00 per year

    Hospice RN Case Manager – Fruitland

    Currently seeking a qualified RN for hospice case management. Case managers provide care where the patient resides, whether that be in a residential area or facility. The candidate will need to have RN license in good standing and should exhibit compassion, empathy, and a professional presence to patients and families we care for.

  • Hospice Volunteer Coordinator - Meridian

    Volunteer services is an integral part of the hospice program and a requirement in serving patients and families. The Volunteer Coordinator manages the hospice volunteer program, including, but not limited to, tasks of recruiting, training, and assignments for volunteers as they provide direct care to patients, families, or offer administrative support to the hospice.

    Location: Merdian home office with travel between several counties routinely with mileage reimbursement.

    Pay: $18.00 – $23.00 per hour

    Job Description:

    · Passion for program development

    · Complete initial assessment with patient and families to determine any volunteer support opportunities

    · Facilitate onboarding and training of new volunteers

    · Assign volunteers to patients and families based on identified needs of patients and preferences of volunteers

    · Participate in routine interdisciplinary group meetings with any updates to plan of care and for ongoing collaboration related to volunteer services in direct patient care

    · Supervise volunteers assigned to patient care or administrative tasks

    · Maintain records related to tracking work time and type of service provided by volunteers to ensure hospice meets required level of activity per Medicare rules

    · Coordinate volunteer recruitment events

    · Maintain documentation of volunteer activities and assignments

    · Continued development of the hospice volunteer program, ie. Initial training, annual education

    · Outreach events and collaboration with community partners for education on hospice services

    · Assist in other day-to-day operations in the office, as assigned

    · Complete annual education as outlined by hospice agency

    Qualifications:

    · Previous hospice/healthcare experience preferred

    · Proficient with electronic medical record system, such as KanTime

    · Strong capacity for collaborative work with a team

    · Meets requirements for employment eligibility for the hospice agency and state/federal laws
